APPOINTMENT POLICY

What to do when you arrive

Upon arrival, we ask that you inform the reception desk that you are here to visit the Exercise Physiologist as they will grant access to the centre and the gym.

Ashburton

Once you have entered the gym we ask that you wait on the seats out the front of consultation room 2 until it its time for your appointment.

Carlton

Once you have checked in at reception we ask that you wait in the welcome area by reception until it is time for your appointment.

Cancelation Policy 

Lifestyle & Health Co is committed to providing exceptional care. When you book your appointment, you are holding a space on our calendar that is no longer available to our other clients. Unfortunately, when one patient cancels without giving enough notice, they prevent another patient from being seen. 

If cancellation is necessary, we require that you email us at admin@lifestylehealthco.com or call us at 1800 943 442 at least 24 hours in advance. A cancellation is considered late when the appointment is canceled less than 24 hours before the appointed time. A no-show is when a patient misses an appointment without canceling. In either case, we will charge the patient the respective consultation fee. 

Appointments are in high demand, and your advanced notice will allow another client access to that appointment time.

Settlement of Accounts

Should the consultation fee be rejected from a third party (insurance, Medicare, DVA, NDIS, etc.) for any reason, the client is responsible for any and all payments of accounts at Lifestyle & Health Co.

Medication 

In cases where you are medically required to carry medication for your condition and safe exercise inclusion such as asthma relievers, nitroglycerin spray, etc, they must be brought along and accessible during each appointment to ensure your own health and wellbeing. 

Home Visits

Generally, we prefer you to come into the clinic so that we have all the right equipment at hand. If this is not possible, regular patients of the clinic who live within the local area can request a home visit. Please note that home visits are booked at the discretion of your practitioner and must be paid for in advance. If the environment is deemed to be unsafe your practitioner has the right to cancel or stop the appointment early. In this case, the client will still incur the full session fee.
